My sef patch dont work. serius problem

sef patch I installed on my joomla 1.5.20 site but does not work....and dont work in 1.5.21 ....  
for some ... for example this article:

BUT EXAMPLE http://www.videoin.eu/film-streaming/1877-harry-potter-e-i-doni-della-morte.html this not ok....

my setup is default:
HTML title settings:    [TITLE] - [SITENAME]
Default title if no title is available:    [SITENAME]
Show META tag title:    No
Show META tag author:    No
Show generator tag:    No
but it is almost always, except in rare cases such as if the SEF patch does not work.

the problem is that if you access the contents of the modules on the home page the link is correct ... if you access by other modules or from the top menu the link is wrong.
sometimes it works and sometimes not?
